Management of Congenitally Missing Mandibular Second Premolars

Second premolars are the most common congenital missing teeth secondarily to third molars, usually associated with prolonged retention, delayed exfoliation and ankylosis of primary second molars. Tipping of adjacent teeth and over-eruption of opposing teeth may be found with premolars missing, resulting in malocclusion of the affected dentition. Treatment of missing second premolars may be divided into 2 main categories: maintain space or close space of primary second molars. The decision depends on multiple factors, including: ankylosis; patient age and gender; the condition of the primary tooth; the orthodontic condition and the condition of the bone after extraction of the primary tooth. Early diagnosis of missing premolars can be obtained in mixed dentition for creating proper treatment plan, leading to higher success rate and lower complications of treatment.
